Our own data from the UMCG transplant program has shown that annually approximately 20% of the potential donor lungs offered are declined because of signs of a respiratory infection; i.e pneumonia due to aspiration-induced lung damage (AILD). The donor organ pool might be increased by accepting infectious donor lungs, treating them and possibly implanting them. There is scarce data on the incidence, pathophysiology and treatment of AILD. Aim of the study This review aims to summarize and evaluate the evidence on pathophysiological mechanisms and therapeutic strategies for aspiration pneumonitis and pneumonia. This helps us also understand where gaps in the literature lie and where potentially new strategies can be implemented. |

Search strategy A systematic search will be conducted for papers published between until January 2024 on PubMed, EMBASE, Web of Science, and Cochrane Library databases, using a pre-defined search strategy. The search string will be built with the help of a professional librarian from the CMB. Included will be RCTs and prospective studies, retrospective cohort studies (with prospective collection of data) in humans, and animal studies separately. The results from the literature search will be uploaded to Rayyan QCRI, a web-based software management program. Two reviewers will independently perform the study selection, data extraction and quality assessment. The Grading of Recommendations Assessment, Development and Evaluation working group approach (GRADE) will be used to assess the quality of evidence. A narrative approach will be taken because the data will be too sporadic and mixed to allow for a meta-analysis.
